Rehabilitation applications

In order to obtain funding for a rehabilitation stay, the completed application should be downloaded and submitted to the Foundation's office or by e-mail.
Individuals with disabilities or seniors over 70 years of age and their
monthly income does not exceed PLN 2600 gross. Assistance under the programs may be provided in the form of co-financing for rehabilitation in one of the 3 centers indicated by the Foundation.

We would like to thank all applicants for their trust. All applications sent to the Foundation are carefully analyzed by us. Unfortunately, due to the number of requests and limited financial resources,
we are unable to help all applicants. That is why the Foundation contacts only people who have decided to provide support in the form of co-financing for a rehabilitation camp.
